Need Your Medical Records?
CHRISTUS Health patients can request physical copies of their medical records by phone or fax by contacting a member of the Health Information Management Department. Phone and fax numbers for the appropriate facility you received care at are listed below. A member of our Release of Information team will be glad to assist you.
Request via fax and mail must be accompanied by your current ID and our authorization for use and disclosure of protected health information form.
You also have the option to request your medical records online. Please click the link to the right to register and/or access your facilities patient portal.

Processing time for request are:
- 15 business days for legal requests
- 15 business days for insurance/HEDIS
- 3 days for facility
Patient requests are fulfilled while patient is in our office, unless otherwise notified that we cannot fulfill it the same day.
Record requests are processed in house. We will either mail or fax them out depending on how many pages there are.

To request your medical records, please download and print a Patient Request form.
To authorize another party to have access to your medical records please download and print an Authorization for Disclosure of Protected Health Information form.
If you would like to have an Advanced Directive implemented please download and print an Advanced Directive form, fill it out, but do not sign it. There are notaries in our HIM department that will assist you with this at no charge.
The advanced directive in Spanish will include the same process. Please download and print this form, do not sign the document, and bring it to our HIM department to complete the notarization at no charge.

Frequently Asked Questions
Medical records are released to the patient of record and/or their legal representative(s). If you wish to have someone pick up your records, their name and address must be listed on the “Send/Release Information” section of the Authorization of Uses and Disclosure of PHI Request Form. They will have to present a government-issued photo identification in order to receive the records.
We may charge a reasonable fee for the cost of copying, supplies and/or postage. Fees are established by State and Federal Guidelines and are updated regularly. If you have any questions, please contact us.
In person requests may take approximately 30 minutes depending on the number of customers waiting. For all other requests, we will respond within timeframes established by State and Federal guidelines. If you have any questions, please contact us.
Records will be provided to you in the format requested on the Authorization of Uses and Disclosure of PHI Request Form (i.e., paper, CD, electronic portal, email, etc.)
