Breast Lift (Mastopexy)

A breast lift, also called a mastopexy, is a common plastic surgery procedure that can quickly and dramatically improve the appearance of the breasts. Pregnancy, breast-feeding, weight fluctuations, aging, gravity, and genetics can affect breast volume, firmness, and symmetry. A mastopexy can help restore a more youthful breast shape by increasing fullness and projection, elevating the nipple position, and improving symmetry—often resulting in enhanced self-image and confidence.

Consultation

During your consultation, your medical history, drug allergies, current medications, and smoking history will be reviewed, and a detailed breast history will be obtained. For most patients, a mammogram before surgery is advisable. You and your provider will discuss your reasons for considering a breast lift, your expectations, and your desired outcomes.

Your provider will examine your breasts, take measurements, assess skin quality, and evaluate nipple position. Based on these findings, you will review options for breast enhancement. The various mastopexy techniques will be explained, including the surgical approach, likely outcomes, and any risks or potential complications.

The Surgery

Breast lift surgery is usually performed on an outpatient basis under general anesthesia. The position of the breast on the chest wall is elevated by removing excess skin and tightening the surrounding tissue to reshape and support the new contour. Breast lifts can be performed using different incision patterns and techniques. The approach chosen may depend on:

  • Breast size and shape
  • The size and position of the areola
  • The degree of breast sagging (ptosis)
  • The amount of excess skin
  • The quality and elasticity of your skin

Some incision lines can be concealed within natural breast contours, while others may be visible on the breast surface. Although scars are permanent, they usually fade significantly over time. Drains are typically not required, sutures are usually absorbable, and dressings can often be removed within 24 hours.

Breast Lift With Implants (Augmentation Mastopexy)

For patients who want a lifted shape and added fullness, a breast lift can be combined with breast augmentation. This may be done during a single operation or staged as two separate procedures, depending on your anatomy and goals. A combined approach can:

  • Restore upper-pole fullness and improve overall projection
  • Enhance symmetry while correcting sagging
  • Provide longer-lasting volume compared with a lift alone

Your provider will discuss whether a single-stage or staged plan is the safest and most effective option for you, along with the placement of the implant (above or below the muscle) and incision pattern needed to achieve your goals.

Types of Implants

  • Saline implants: Filled with sterile salt water. They can be adjusted for minor size differences and typically require a slightly smaller incision.
  • Silicone gel implants: Filled with cohesive silicone gel for a more natural feel. Periodic imaging may be recommended over time to assess implant integrity.
  • Highly cohesive (“gummy bear”) implants: Form-stable implants designed to hold their shape; may be helpful for patients seeking more upper-pole stability.

Implant size, shape (round or anatomical), and profile are selected based on your chest width, soft-tissue characteristics, and desired outcome. Your provider will also review typical considerations such as implant longevity, potential need for future replacement, and risks like capsular contracture or implant rupture.

Recovery

Most patients return to work within a few days. Breast soreness typically subsides after 1–2 weeks, and exercise can usually be resumed in about 4 weeks. Results are visible immediately, with swelling decreasing over time and incision lines fading. Satisfaction with the new breast shape often increases as recovery progresses.
